Medical examination and preparation of documents for a Canadian visa

Dr Olivia Caune provides a full medical examination and prepares the documentation required for submitting a Canadian visa application. The service includes a detailed assessment of your state of health, the performance or evaluation of the necessary laboratory tests, as well as the preparation of a doctor’s report and medical certificate in English in accordance with the requirements of the Canadian immigration authorities.

Scope of the service:

• A detailed analysis of the patient’s medical history and a physical examination;
• Ordering and interpreting the necessary laboratory tests (blood tests, X-rays, ECGs, etc., as required);
• Preparation of doctors’ reports and medical opinions in English;
• Completing forms and compiling documents in accordance with Canadian visa requirements.

How to prepare for a doctor’s appointment when applying for a Canadian visa

• A copy of your passport
• Vaccination record
• IME number (if applicable), visa type
• Deadline for submitting documents
• Address, telephone number, email

You must bring the following with you to your appointment:

• A passport that is still valid
• Vaccination record
• A statement from your GP regarding past illnesses (diagnoses only), operations and any medication you are currently taking.
• medicines
• For women aged 18 and over, a doctor’s certificate confirming a gynaecological examination within the last 12 months.
• A maternity card for pregnant women.
• Glasses, if you wear them.

An X-ray costs 60.00 euros; vaccinations (if required) and laboratory tests are paid for separately in accordance with E. Gulbja’s laboratory www.egl.lv  price list.

The medical examination takes place in person at Jūrmala Hospital, room A104, 19/21 Vienības prospekts, Jūrmala.
