What is included in the initial and follow-up consultations
If you have noticed any changes in how you feel – persistent tiredness even after a good night’s sleep, unexplained weight fluctuations, sleep disturbances, a rapid heartbeat or excessive sweating, – this may indicate a disorder of the endocrine system. Many patients spend months seeking answers from various specialists, unaware that the root cause of these symptoms lies in a hormonal imbalance. Dr Olivia Caune, with over 13 years’ experience in endocrinology and general medicine, will help you understand the true causes of your symptoms and develop the most effective treatment strategy, which will improve your quality of life from the very first appointment.
The initial consultation comprises the following stages:
- A detailed summary of the patient’s medical history – chronic conditions, family medical history, previous investigations, medication and dietary supplements taken;
- Physical examination focusing on the endocrine system – palpation of the thyroid gland, assessment of the condition of the skin and hair, auscultation of the cardiovascular system, and assessment of signs of peripheral oedema;
- The initial diagnostic plan – ordering blood tests in accordance with current guidelines (TSH, free T4, free T3, glycated haemoglobin HbA1c, glucose, insulin, C-peptide, calcium, vitamin D, parathyroid hormone, cholesterol fractions);
- The development of a personalised treatment plan with clear next steps, choice of medication and dosages, as well as lifestyle recommendations.
A follow-up consultation is just as important – it is designed to assess the effectiveness of your treatment, adjust your medication doses in line with your test results, and answer any questions you may have. Regular medical monitoring is particularly crucial for patients with type 1 and type 2 diabetes, autoimmune thyroid disorders (Hashimoto’s thyroiditis, Graves’ disease), osteoporosis and metabolic syndrome. Dr Caune works in collaboration with the laboratory and diagnostics department at Jūrmala Hospital, which ensures rapid and accurate diagnosis under one roof, so you do not need to travel to multiple facilities.
In Dr Caune’s practice, it is common to encounter patients with undiagnosed thyroid dysfunction who have been treated for depression or cardiac arrhythmia for a long time, without realising the role played by their hormonal status. Similarly, many patients with type 2 diabetes are able, following their initial consultations, to normalise their blood glucose levels and reduce or completely discontinue their oral medication, thanks to a personalised diet and exercise plan. Every body is unique, so treatment cannot be one-size-fits-all – it is precisely this approach that distinguishes high-quality endocrinological care.
